On this page, we’ve provided you with the PDF forms and photo evidence we’ll need to assist you in getting a duplicate title.
ORIGINAL DOCS ARE REQUIRED!
Original docs are the title (pink slip), power of attorney, and all of the forms you have printed, filled out and signed (REG 227, REG 256, REG 5103), as well as printouts of the required photographs.
All of these must be physically mailed or overnighted with wet ink (no copies) to:

Once you’ve mailed all of the required original documentation, we will contact you to process your credit card payment before we can proceed.

The Forms You’ll Need to Fill Out:
- REG 227 Request form for duplicate title.
- REG 256 Statement explaining any extenuating circumstances related to this.
- REG 5103 Application for temporary smog exemption for a vehicle located out of state.
NOTE: We fill out the REG 31 Vin Verification form.
Photo Evidence Required:
- Photo of VIN in front window
- Photo of VIN in driver’s door frame
- Photo of odometer
- Photo of license plate
- Photo of emission sticker (under the hood)
- Photo of your ID
- Power of Attorney (if applicable)
- Last registration document (not a photo or copy)
